Paperless FAQs
Has my clinic been activated for Paperless collections?
As of June 2025, many clinics have had Paperless CCFs activated. Other clinics will be activated on a group-by-group basis over the next couple of weeks. Rollout dates are being communicated via email to clinic administrators.
Will I have to do anything to configure this at my clinic?
No, all changes will be made on the FormFox and CRL side.
Do I need to opt in for my clinic to be turned on?
No. Once your clinic has been activated, you will no longer print and include the lab copy of the CCF with the specimen shipment to the laboratory for all Non-Federal CRL collections.
Is any additional training needed?
No, the collection process will remain the same. However, you will no longer print and include the lab copy of the CCF with the specimen shipment to the laboratory. The change will apply to non-DOT collections. DOT collections will not change.
Will we be notified in advance so that we can communicate the specifics to our field and collectors?
Yes, the clinic administrator on file will receive an email notification before your clinic goes live for paperless.
Will there be any changes in the application - cosmetic or warning messages that are presented?
Yes, the header in the workflow will read “FormFox Electronic Lab Copy” and there will be warning text on the print page stating that the lab copy is distributed electronically.
Can I still print a CCF copy before finishing the event or render some version of it on the completed record?
Yes, clinics still have the ability to print copies 2 – 5 of the CCF.
Should I print a screenshot of the completion page and send with the sample?
No, extra documents should not accompany the specimen shipment to the laboratory. Any extra documents may flag the sample as having a defect at the lab.
Are any states excluded from this change?
Due to state regulations, DFW collections in Tennessee, Florida HRS collections, and collections performed in the state of Hawaii will be excluded from this change until a later date.
