SMS/Text Message Notifications Terms of Use

  1. Program Description
    Clinical Reference Laboratory, Inc. (“CRL”) may use messaging to notify you about upcoming appointments, the status of your events and orders (e.g. that a collection kit has been shipped to your house, or that a sample has reached the laboratory for testing), and the availability of reports. We also use messaging to uniquely identify you by sending one-time use PINs.

  2. Message Frequency
    Message frequency may vary. Messages are generally sent one time per event.

    For example, once when the lab receives your sample, once when your report is available, and once each time you log into a CRL website.  In some limited number of cases, a limited number of follow-up notice may be delivered one to five days apart. For example, notifying you immediately after scheduling about an upcoming appointment, and then 24 hours before that appointment as a reminder.

  3. Message and Data Rates
    Message and data rates may apply to any SMS/Text messages sent or received in connection with CRL text message notifications. Check with your mobile carrier for details regarding your messaging or data plan.

  4. Opt-Out Instructions
    You may opt out of receiving SMS/Text notifications at any time by replying STOP to any CRL text message. After you send the STOP message, you will receive a confirmation message and will no longer receive SMS/Text notifications from CRL unless you re-enroll.

  5. Help Instructions
    If you need assistance with the SMS/Text notification program, reply HELP to any message or contact CRL Customer Support at 1-800-882-1922 or Accessmyhealth@crlcorp.com.

  7. Carrier Liability
    Mobile carriers are not liable for delayed or undelivered messages.
